Ramps rather than elevators, and they are on opposite blocks: the Coney Island-bound ramp is at East 15th St and Avenue H, the Manhattan-bound ramp at East 16th St and Avenue H. Choose the side before crossing, because there is no step-free crossover between the two. OpenStreetMap still tags the platform stop as not accessible, while the MTA lists both ramps. Buses are the B49 and B68.
